If something breaks, please advise engineering, and we will fix it as soon as possible. If you do not tell us, it remains broken, and in some cases, it gets worse. A small problem is a big problem because nobody reported it. We don't point fingers and place blame (publicly). You won't get in trouble if you break something. You will receive my express disappointment if you do not report it. Do you want to experience my disappointed face?

Summary: report when things are broken when they happen or are noticed, not months later.

Maybe we need to have a way that people can submit ANONYMOUS requests!

Just submitting a request should be fine. There is no need for cloak and dagger. We just need to know.

But if they're nervous about getting in trouble maybe not having their name attached would help.

We are all adults here on a ship in the air. We should not need to worry about such things. Just report it and move on. Things break. Unless you are being an idiot there should be no shame in this.

And if you do, you fix it. I just do not see why we need to coddle people. But if you want to implement an anonymous form, I suppose I cannot stop you.
