If something breaks, please advise engineering, and we will fix it as soon as possible. If you do not tell us, it remains broken, and in some cases, it gets worse. A small problem is a big problem because nobody reported it. We don't point fingers and place blame (publicly). You won't get in trouble if you break something. You will receive my express disappointment if you do not report it. Do you want to experience my disappointed face?

Summary: report when things are broken when they happen or are noticed, not months later.

yeah, person who noticed a live wire whipping in a corridor, tried to CATCH. IT. WITH THEIR HANDS. and then told us a HALF HOUR later after they were released from the medbay.

mine is about 50 cm but i've helped catch bigger ones that were just as difficult
